Department of Clinical Pharmacy

General Description

The emergence of the concept of clinical pharmacy has been closely related to the inevitable developments experienced in hospital pharmacy since the 1960s. Until the mid-1960s, hospital pharmacists mostly engaged in traditional pharmacy activities such as prescription and drug preparation. However, the increasing diversity and complexity of medicines on the market and awareness of drug mistakes has led pharmacists to step out of the hospital pharmacy and become increasingly involved in patient services. The developments in these services have been stated as a shift from drug-centered pharmacy to patient-centered pharmacy in hospital pharmacy and officially approved as ‘clinical pharmacy’ in the Nuffield Report in 1986. Clinical pharmacists contribute to the coordination of health care services, which is provided to the patients, in health care settings where they interact frequently and regularly with physicians and other health professionals. In addition, clinical pharmacists work directly with doctors, other health professionals, and patients to achieve the best possible health outcomes of ongoing treatment with the prescribed medicines. As an essential member of the multidiciplinary team clinical pharmacists are actively involved in many processes, including reviewing patient’s previously and currently taken drugs, assesment of allergies, taking patient’s drug history, monitoring of response to the treatment, discharge of patient, investigation of unwanted effects and drug interactions.
